Coexistence of Growth Hormone-Secreting Pituitary Adenoma and Internal Carotid Artery Aneurysm

The co-existence of pituitary adenomas and cerebral aneurysm is rare, although the prevalence is higher than would be expected in the general population. We report a rare case in which a growth hormone-secreting adenoma co-existed with an internal carotid artery aneurysm. A 68-year-old woman presented for evaluation of a headache. On physical examination, acromegalic features were noted. Magnetic resonance imaging revealed a pituitary macroadenoma, which was associated with an internal carotid artery aneurysm. Complete endovascular obliteration of the aneurysm was achieved using Guglielmi detachable coils. Total resection of the pituitary adenoma was performed via a transsphenoidal approach 2 weeks later. Elevated growth hormone and IGF-1 levels or their biological effects might be implicated in the genesis of an aneurysm. The treatment of this dual-pathology could be carried out in two steps; the vascular pathology should be treated first to avoid potential future catastrophic hemorrhage and the pituitary pathology subsequently.
